Consumer Information Reports

There is never a fee for our reports or assistance. We ask that if you are able please make a donation to help cover the cost of keeping our information current, and to generate, print and mail the reports.

When ordering, be sure to include your name, mailing address and telephone number on all correspondence.
Reports contain:

  • Facility name, address and phone
  • Current and past administrator(s)
  • Management/ownership history
  • Three most recent inspection survey dates
  • Total number of deficiencies cited each year, including substantiated complaint deficiencies
  • Number of deficiencies cited as actual harm, mistreatment, or immediate jeopardy to residents
  • Awards and enforcements for the past five years
  • Private pay daily rates
  • Number of full time and part time licensed nurses
  • Occupancy rates for nursing homes and assisted living
  • Whether or not the facility has a special care unit for residents with Alzheimer’s or dementia
  • Annual inspection and complaint investigation recommended remedies

KABC is the only source in Kansas for detailed and historical information about state licensed Assisted Living Facilities, Home Plus, Residential Health Care Facilities, and Boarding Care Homes in Kansas.

  • Number of full time and part time licensed nurses

  • Enforcement actions (fines and other penalties) for the past five years
  • Annual inspection and complaint investigation summary for three years
